چکیده

Background: Volatile anesthetics inhibit cardiac transmembrane ionic currents and intracellular ionic activity and exert antiarrhythmic actions. Among electrocardiogram (ECG) waveforms, the effect of volatile anesthetics on U wave has never been studied. The aim of the present observational study was to evaluate the possible effect of desflurane on U wave in anesthetized patients. Methods: Peri-operative ECG tracings (lead II) were recorded and analyzed from 24 gynecologic patients (American Society of Anesthesiologists (ASA), class 1). Anesthesia was routinely induced with thiopental, fentanyl and succinylcholine and maintained with desflurane (1 to 1.5 minimal alveolar concentration (MAC)) and O2. U wave amplitude (Uamp) was manually measured from amplified ECG records. Results: Discernible U wave in 24 patients varied in amplitude (64 24 V). U wave was three times larger when heart rate increased to maximum during tracheal intubation. Similarly, Uamp was augmented (285% 58% of the control) in extrasystolic beats elicited by tracheal intubation. Desflurane (1 to 1.5 MAC) significantly and reversibly suppressed Uamp (a decrease by 60 24 V, n = 24, p < 0.05). Conclusions: The suppressive effect of desflurane on Uamp might be explained by its inhibitory effects on transmembrane ionic currents, intracellular calcium load and delayed afterdepolarizations. Our results support a contributory role of afterpotentials in the genesis of U wave.

Effects of Repeated Application of Isoflurane and Desflurane on Electrocardiogram, Anaesthesia Induction, and Recovery Characteristics in Rats

Thirty male Wistar albino rats were equally divided into two groups. All the animals were sedated with 5 mg kg of xylazine hydrochloride, and then 2.5% isoflurane or 8% desflurane with 100% oxygen by mask induction were given and anaesthesia maintenance was continued for 60 min with 5.7% desflurane or 1.4% isoflurane. Anaesthesia applications were repeated on the 1, 3, and 7 d in both groups. H...

Origin on the electrocardiogram of U-waves and abnormal U-wave inversion.

AIMS Soon after the initial development of electrocardiography, U-waves were discovered in many normal subjects following the T-wave repolarisation waveform on the electrocardiogram. Various explanations have been offered for their origin, but none is universally accepted. We used our model of left ventricular repolarisation to explore the most common hypotheses for the genesis of U-waves. ME...

Evaluation of Electrocardiogram Changes in Patients with Slow Coronary Flow

Background: Slow coronary flow (SCF) is defined as a condition in which in spite of no evidence of coronary occlusion or stenosis in angiography, there is delayed pacification of the vessels after the injection of a contrast agent. There are several studies about electrocardiogram (ECG) changes in these patients, but changes like OLD MI, RBBB, LBBB, LVH and ST-T wave have not b...

The Double U Wave—Should the Electrocardiogram be Interpreted Echocardiographically?

UNLABELLED The U wave is still an electrocardiographic deflection of enigmatic origin. Numerous hypotheses on its origin have been formulated, but to date none has been conclusively proven. Recently, a report described the first case of bifid (or notched) U waves. Until then this phenomenon has only been described in the T wave. This is the first report of double U waves-two separate deflection...

The enigmatic sixth wave of the electrocardiogram: the U wave.

The U wave is the last, inconstant, smallest, rounded and upward deflection of the electrocardiogram. Controversial in origin, it is sometimes seen following the T wave with the TU junction along the baseline or fused with it and before P of the following cycle on the TP segment. In this review we will study its temporal location related to monophasic action potential, cardiac cycle and heart s...
